Alumni Continuum Program

Duration: 12 Weeks
Meetings: Weekly one-on-one sessions

12-week continuing care program built around eight skills that research consistently connects to long-term recovery and well-being. Each skill builds on the one before it. Together they give people a practical framework for living, not just staying sober.

  • Alumni transitioning out of residential or outpatient treatment
  • Individuals needing continued structure and accountability post-discharge
  • Clients at risk of losing momentum without ongoing support
  • Those ready to apply recovery principles in real-world environments

Includes:

  • Twelve weekly one-on-one coaching sessions
  • Companion workbook with structured, guided reflection, and tools grounded in behavioral science
  • Evidence-based mindfulness practices to support present moment awareness and stress regulation.
  • Behavior change strategies to grounded in habit science to reinforce healthy routines
  • Practical, real-world tools for integrating recovery principles in daily life
  • Ongoing engagement and accountability between sessions

Ongoing Support:

Optional continued one-on-one coaching is available for alumni who want extended structure and accountability beyond the initial 12 weeks.

Extending Support Beyond Discharge

Some residents benefit from continued guidance as they transition back into daily life.

I offer optional one-on-one Mind-Body-Spirit training for residents after discharge. These sessions help reinforce the regulation skills they began developing during treatment and support their transition

into everyday life.

Many clients can use HSA or FSA funds, making continued support accessible and affordable.
